Notice to All Customers: Pursuant to Ohio Revised Code 3705.29, it is unlawful to purposely obtain, possess, use, sell, furnish, or attempt to obtain, possess, use, sell, or furnish to another for the purpose of deception any certificate, record, or certified copy of it that relates to the birth of another person, whether living or dead.
We issue certified copies of Ohio birth records (from 12/20/1908–present) and Darke County death records.
How Much Do Certificates Cost?

What to Bring
✅ Completed Birth Certificate Application or Death Certificate Application
✅ Valid photo ID (required if requesting SSN on death certificate)
✅ All available information for the person on the record (full name, date of event, parents’ names, etc.)
How to Order
🕘 Walk-in Hours:
Monday–Thursday: 7:30 AM – 5:00 PM
No appointment needed. Closed Fridays. Pay by cash, check, or credit card. (Credit cards accepted until 3 PM)
📬 Order by Mail: Include application, copy of ID, and payment (cash or check)

To include the Social Security Number on a death certificate, you must qualify and provide ID.

Home Births: Print a home birth packet or pick one up from our office before delivery. Submit within 10 business days after the birth.

Exchange Policy for Birth and Death Certificates
We offer free exchanges for birth and death certificates within one (1) year of the original purchase date. To be eligible, the request must include the original certificate. After one year, standard fees may apply.
