Shot Records
Need a copy of your immunization record? Complete the Release of Information Form and return it in person or fax to 937-548-9128.

Insurance & Fees
We accept Medicaid, Medicare, and many private insurance plans.

• Uninsured children: Free vaccines through the Vaccines for Children Program ($20 administration fee applies).

• Uninsured/underinsured adults: $20 per dose (max $80).
